Community Programme Feedback Questionnaire

We would appreciate your help in giving a couple of minutes to complete the following questionnaire. This will enable us to ascertain the impact of the Community Programme to date and whether improvements can be made. Please answer all questions as honestly as possible.

If you wish your answers to remain confidential you do not need to fill in your name, however please do complete the division and location sections as this will enable us to gauge the level of involvement per site/location.
